ROSTER & WAIVER FORM
Prior to participating in each Titan Tournaments event, your team must complete the roster and waiver form included in the attachment below. We also require that your team submit proof of team insurance at each event, with Titan Tournaments, Inc. listed as additional/co-insured.
That should look something like this on your insurance paperwork:

All roster and waiver forms, along with team insurance shall be submitted in hard copy form to the on site Titan Director prior to first pitch at every tournament. ALL FIELDS MUST BE COMPLETED. If you cannot locate the director please locate their phone number where scores are posted on site and contact them, so you can turn in your roster and waiver form, and team insurance. Please make extra copies of your roster/waiver form, and your team insurance, as you will need to turn these in at every tournament.
