ROSTER & WAIVER FORM

Prior to participating in Titan Tournaments events, your team must complete the roster and waiver form included in the attachment below.

All roster and waiver forms shall be submitted in hard copy form to the on site Titan Director prior to first pitch. ALL FIELDS MUST BE COMPLETED. If you cannot locate the director please locate their phone number where scores are posted on site and contact them, so you can turn in your roster and waiver form. Please make extra copies of your roster and waiver form, as you will need to turn one in at every tournament.
